ページャーを使用すると<p class="page_links"> </p>が勝手に挿入されますが今回のマークアップでは<div class="page_links"> </p>を使用するので挿入されないようにする必要性があります。

お問い合わせフォーム周りを整備しました。Bootstrapのマークアップに従って、送信画面、確認画面、完了画面をマークアップしています。ラジオボタン、チェックボックス、セレクトボックス等に関してもマークアップ済みです。
